RYS - Booster Club Registration Information   Registration Forms All RYS-Booster Clubs whom are registering football, baseball, softball, or basketball teams must complete and submit the appropriate roster & forms by each set deadline.  The following is a list of forms that correspond to each sport:  Rosters Football Cheerleading   Baseball Softball  Basketball  17 & Under Basketball Rosters       Other Forms to be Filled Out and turned into HCPL with Rosters   Adult Liability of Waiver Form (per coach)   Liability of Waiver Form  (per player)   Insurance  (per player) Playing Up Form Player Release Form Emergency Medical Form  (Coaches keep this form) Background Consent Form   Volunteer Coaches Form (Each booster club needs to provide HCPL with this list of approved coaches for your organization on the volunteer coaches form.)   printer-friendly
